Support as an adult with SEND

​18 onwards

On the day of the young person’s 18th birthday support will be transferred from Children’s Services to Adult Services. After the young person has been using adult services for three months, the transitions worker will hold a review meeting to discuss the move to adult services and the package of support that has been provided.

Everyone who is involved in the young person’s care and support will be invited to attend this meeting. If everything is going well, the transitions worker will make a formal referral to adult services to confirm the move. The package of support that is being provided to the young person will be reviewed once a year by the young person’s adult social worker after this point.

If the move to adult services is very complicated, a member of the transitions team may remain as the young person’s social worker until the move to adult services is complete or the young person reaches the age of 25. The transitions worker will discuss this with the young person and their family or carer before a decision is made.
